Instructions for Completing and Submitting the Player Profile Form

To create your Player Profile for college recruitment, please follow these steps carefully. This profile will be sent to prospective colleges, so ensure all details and photos are accurate and professional.

Step 1: Download the Player Profile Form

  1. Click the link to download the Player Profile Form: HERE

  2. Save the form to your device (computer, tablet, or smartphone).Step 2: Fill in the Form Online

    1. Open the form using a PDF editor (Adobe Acrobat, Microsoft Edge, or Preview on Mac).

    2. Complete all sections with accurate and up-to-date information.

      • Ensure stats, achievements, and academic details are correct.

      • Use proper grammar and spelling (this document represents you).

    3. Save the completed form as:
      “FirstName_LastName_PlayerProfile.pdf”
      (Example: John_Smith_PlayerProfile.pdf)

    Step 3: Take a Professional Player Headshot

    A clear and professional headshot is required for your profile. Follow these guidelines:
    ✅ Use a smartphone or camera (ensure high resolution).
    ✅ Plain, light-colored background (white, light gray, or beige).
    ✅ Centered face, full view (no hats, sunglasses, or heavy shadows).
    ✅ Neutral facial expression (a slight smile is acceptable).
    ✅ Well-lit and in focus (avoid dark or blurry images).

    📌 Tip: This should resemble a passport photo, but you may wear your team jersey if preferred.

    Step 4: Email the Form & Headshot

    1. Attach the completed Player Profile Form and headshot photo to an email.

    2. Subject Line: “Player Profile – [Your Full Name]”
